Your questions make up the most important aspect of your survey. With Survey, you have several different question types to choose from, giving you the ability to customize your survey the way you want.
 

1 Access the Survey Designer. Begin by selecting your survey. From the left-hand navigation, click Design, then Survey Questions.

2 Add a question. In the top left of the canvas, click the dropdown, and in the Survey section, select the survey you want to add your questions to if you have multiple chapters.


To the right-hand side of the designer, in the Questions section, click and drag the type of question you want to create onto the canvas. Question types include:
  • Choice Question allow respondents to select from a list of pre-determined options. Display choices include a single answer list or multiple answer list.
  • Date & Time Question allow respondents to enter in a specific date and time. Display choices include both date and time, or date only.
  • Form Question allow respondents to enter a list of answers, such as their personal information or a list of their favorites.
  • Image Question allow respondents to choose an image choice from a pre-determined list.
  • Matrix Question allow respondents to select from a list of pre-determined options that apply to multiple categories. Respondents will be able to select a single response or multiple responses for each category.
  • Matrix Rating Question allow respondents to provide their rating to multiple categories.
  • Matrix Spreadsheet allow respondents to provide answers to multiple questions and categories. Different answer types can be collected, such as a text response, phone number, email address, number, currency, or dropdown selection.
  • Net Promoter Question allow respondents to enter select their willingness to recommend the item in question on a case from 1-10, such as how happy a respondent is with a product.
  • Number Question allow respondents to enter a specific numerical value. Display choices include numbers, currency, or decimal responses.
  • Rank Order Question allow respondents to provide a value to different choices based on the particular scale.
  • Rating Question allow respondents to provide a value to the question based on the particular scale.
  • Text Question allow registrants to type out any general text. Display choices include a text field or comment box.
Depending on the question you add, the configuration options may vary.

In the Question Text field, enter your question, then if necessary, add Instruction Text that will assist your respondents. Establish your label placement for the necessary questions. If you want the question to be required, toggle "Required" to blue.

In the Response section, set up the choices.

3 Set up visibility logic, if necessary. If you need to set up visibility logic, make sure the question you want to appear based on another question's answer is on another page. If they appear on the same page, the second will not appear, even if the correct value is selected. To set up the logic, scroll down to the Logic section, and click Visibility Logic
Use the dropdowns to establish up to five criteria.

4 Add link logic, if necessary. Link logic lets you personalize a question's answers based on a previous question's response within your survey and custom fields. To set up the logic, scroll down to the Logic section, and click Link Logic.
 
NOTE: Have a lot to add? Select Set up logic using Excel to export all the choices, make your updates, and then import your logic in bulk.

Click Create logic. Choose your source question from the dropdown. Click Add Logic, then, from the dropdown, select your answer choice. To the right, click the dropdown, and select the answers that will display.

Repeat this for each choice. Click Apply.
 
Click Save to preserve a draft or Publish to make it visible to respondents.
